Confirm CGM requirements
Before submitting the referral, confirm that your patient is:

Insulin treated OR
-
Has documented problematic hypoglycemia

The patient must meet ONE of these requirements:
OPTION 1
The patient uses insulin
OPTION 2
The patient has a history of problematic hypoglycemia
with documentation showing:

More than one blood sugar reading below 54 mg/dL that continued despite attempts to adjust treatment
------- or -------
-
One blood sugar reading below 54 mg/dL that caused the patient to need help from another person, such as passing out or needing emergency care.

Include the patient's diabetes diagnosis code.
